Subject access requests
The Data Protection Act 2018 and UK GDPR provides the following rights for individuals;
The right to be informed:
- We meet this by providing you with a Privacy Notice, it tells you:
- about our commitment to meet the requirements of the Data Protection Act
- why we collect information about you
- what information we collect about you
- in what format your information is held
- the legal basis for collecting information about you
- how your information is used
- how we keep your information safe
- when we might share information about you
- your rights
- how this affects you
The right of access:
You have the right to make a written request to either view or request a copy of your health records
The right to rectification:
You have the right to have inaccurate information about you corrected or removed
The right to erasure (Right to be forgotten):
You have the right to have certain personal information about you erased
The right to restrict processing:
You have the right to request that your personal information is only used for restricted purposes
The right to data portability:
You have the right to ask for the personal information you have made available to us to be transferred to you or a third party in machine-readable formats
The right to object:
- *where our processing is based on the performance of a task carried out in the public interest
- *where the processing is necessary for our or a third party’s legitimate interests, or for direct marketing
Rights in relation to automated decision making and profiling:
*You have the right not to be subject to a decision based solely on automated processing
